How We Work with Case Managers

Our team of advisors regularly provides case management support, advising on, organising and implementing seamless packages of domiciliary and live-in care for individuals across the UK. We’ll work with case managers to establish the individual needs of their clients.

We regularly receive and process enquiries from case managers responsible for both pre and post-settlement cases. It’s commonplace in medical case management, for a care case manager to need initial care package costs and conditions to support the litigation process.

In these circumstances, our advisors can work quickly and effectively to determine and deliver comprehensive and fair quotations, which can then be presented to solicitors. We’ll conduct assessments over the telephone or face-to-face.

For case managers working with post-settlement cases, we can establish their clients’ needs and implement bespoke packages of care in accordance with personal time scales and appropriates. Once these packages have been implemented, our locally-based managers and fully-trained carers will work alongside case managers to ensure that the best outcomes are achieved.

What Can A Care Case Manager Expect When Working With Helping Hands

The team co-operates closely with case managers to ensure they get exactly what they need when organising bespoke specialist clinical or complex care. They can expect:

Complex Case Management Support – Care Case Assessments

Having established the needs of case managers and their clients, we’ll assess their personal needs before writing a recommended care package. This package covers both physical and personality requirements.

These complex care case assessments are carried out by our senior registered nurse specialist, Melanie Dawson, and a team of locally-based care managers with expertise in specific condition-based care. They’ll visit prospective clients to draw up an individual Detailed Support Plan (DSP).

Following this assessment, we’ll write this recommended care package. During this stage we can also advise and organise any specialist equipment or room adaptations, select a suitably-trained carer who’s the right personality match, establish final quotations and discuss essential budget projections.

We see all of the people we provide packages of care for as what they are – real people with unique personalities. This is why, ensuring clients are paired with a carer they share common interests with and a similar personality is something we see as vitally important. Our experts work with case managers and legal professionals to tailor our services in the most cost effective manner to accommodate those all-important budgets. Meanwhile, our robust accounts team ensure that financial administration is conducted effectively and efficiently too.

As required, we keep all records up-to-date, and they’re amended on a per case basis depending on specific requirements from case managers. Our role, as part of the client’s professional ‘care circle’ is to provide transparency of information and assist our case management team to build positive and efficient working relationships.

Outcome-Based Care Solutions

With our care case management services, we take an outcome-based approach in order to deliver individually-tailored packages of care. We use DSPs which comprise personal outcomes – these comprehensive plans enable us to provide person-centred care that meets the exact needs of clients.

Before proposing or implementing a package of care, we’re more than happy to present detailed profiles of our carers and adapt our assessment phase to meet the preferences of our clients. The team takes a meticulous approach to matching clients and carers, with matches based on nursing-led skills, experience and personality.

Tailored Medical Assistance

Once our team has established a plan of support, which has been reviewed by the care case manager, the provision of the tailored care package can start. It doesn’t matter whether the client requires 24-hour support (live-in or away), or hourly visiting support, our trained carers can meet their complex and social care needs.

We can provide support for clients affected by a range of conditions, including multiple sclerosis, cerebral palsy, spinal injuries, acquired brain injuries, motor neurones disease and much more. Our carers can also deliver complex medical assistance with bladder and bowel management, PEG feeding and management, insulin administration, BiPAP and CPAP care, continence care, and single and double-up hoisted transfers.

We work with healthcare and multidisciplinary teams (MDTs) to manage and monitor complex care cases. Before going to provide care in a client’s home, our carers are trained to proactively monitor, observe and report changes in clients’ conditions. Our care case management service is designed to be flexible and offer adaptability.

Care Case Management – Continuity

Continuity in care is important, and it’s something we’re highly committed to. We strive to assign one named carer with specific expertise relating to each case wherever possible. We also guarantee seamless cover for carer breaks, with 24-hour handovers completed as standard.

Our local care managers conduct regular visits and telephone calls, where clients can raise ideas or voice concerns, which we can address in a timely manner.
