| Live in care is often appropriate to facilitate a prompt and safe hospital discharge, which at a later date can be reduced as the patient re-adjusts to being back at home. Our assessors are more than willing to come out to visit patients in hospital before they are discharged, so that a service can be agreed. Two thirds of all our live in clients have been referred via Social Services. Other sources of referral include Word of Mouth, GP’s and Age Concern. |
