Live in care is often appropriate to facilitate a prompt and safe hospital discharge, which at a later date can be reduced as the patient re-adjusts to being back at home.  Our assessors are more than willing to come out to visit patients in hospital before they are discharged, so that a service can be agreed. Two thirds of all our live in clients have been referred via Social Services. Other sources of referral include Word of Mouth, GP’s and Age Concern. Helping Hands Home Care : Click Here
