Preventative care plans

Preventative care plans

What is a preventative care plan?

Preventative care is measures that are put in place to try and prevent someone becoming ill, or their condition becoming worse. It is a proactive approach involving measures such as regular checkups, screenings, and vaccinations.

It is designed to detect potential problems early so that they are easier to treat, or prevent them occurring at all, wherever possible.

By giving information on lifestyle choices and offering medical interventions, it is designed to help people avoid potential health issues.

The type of information you might find in a preventative care plan includes:

  • Vaccinations
  • Health screenings
  • Annual physicals
  • Routine blood tests
  • Lifestyle support
  • Funding information
  • A woman wearing a lilac uniform and blue gloves smiles whilst chopping vegetables, including carrots and peppers, in a bright kitchen. A kettle and a plate of greens are visible on the worktop nearby.

Why should I have a care plan?

Having a care plan is an essential preventative measure because it will detail the type of support you’re receiving, and the measures put in place to keep you as well as possible. Benefits include:

  • Early detection
  • Reduced risk
  • Improved health
  • Peace of mind
  • Two women sit on an outdoor bench covered with a striped blanket, enjoying drinks and smiling at each other. A picnic hamper with plates and cutlery is set between them. A hedge and green grass are in the background.

What does an effective care plan include?

An effective care plan will include information about the person’s life and lifestyle, including:

    Personal information

Such as age, weight, blood pressure, etc

    Health conditions

If any, and how they affect the person’s life

    Medication administration

What they take and when it’s given

    Emergency contacts

Contact details of family or advocates

    Hobbies & interests

What the person likes to do and where

    Goals and review schedule

    How progress will be monitored

How can I create my own care plan?

Creating your own care plan is as easy as saying what you would like and how you want your daily routine to progress. Your caregivers and wider healthcare team will actively encourage you to be involved in your care planning process because person-centred care is always preferred.
