Condition-led care
What is condition-led care?
Condition-led care is dedicated support to help you manage the symptoms of your condition.
Condition-led care is a personalised approach that customises care to meet specific wants or needs. It aims to address challenging objectives where conventional methods have fallen short.
It may be nurse-led with clinical care, and focuses on helping you to manage any symptoms you may be experiencing so that you’re able to continue living comfortably and independently.
