The extensive clinical experience of our nursing team allows us to work in partnership with all the relevant organisations to organise fast-track, 24-hour home care packages for people with complex healthcare needs. This involves working with other organisations on completing and reviewing decision support tools (DSTs) for home care.

CHC teams - what to expect from us
