What is a disability care plan?
Care planning is essential for anyone who needs additional support day-to-day, but when someone is living with a disability it can be even more important to make sure there are effective plans in place for continuity of care. Why is it important to have a disability care plan?
Your wishes and preferences are key when it comes to devising an effective care plan and there should be input from everyone involved in your care, especially your wider healthcare team and, where appropriate, your loved ones. Your circumstances at home will also be relevant to devising an effective care plan, whether you live with your family, on your own, or are branching out independently for the first time and having a carer live with you. You will always be the focus of any disability care plans that are produced and will be consulted at all times to make sure your care plan is working for you. If it isn’t, it will be amended so that your days progress more satisfactorily.
Do I need a care plan?
While you may not think you need a plan, disability care plan examples show that it is an important document that will help to ensure that your care remains consistent. It’s also important that your support is mapped and monitored constantly and that the plan allows for a flexible approach to your care. Having an up-to-date plan in your home means that all of your careworkers know exactly how you wish to be cared for and how best to understand your condition. What should an effective care plan for disability include?
There’s no such thing as an average care plan for a disabled person. Your care plan should be as unique as you are, containing all the information your support team need to keep you at the centre of your care experience. Your care plan may contain a huge amount of detail, but as a minimum will usually include:
Details of your assessed needs
The condition(s) you live with, how you need support
What your goals are
What you are hoping your care will achieve
Any support you have in place
Family, friends, volunteers, etc
Times and days of care provision
During the day, overnight, or around-the-clock
Your hobbies and interests
What you like to do and any classes or groups you attend
Details of your routines
How you like your days to progress
How I can create my own care plan
If you are receiving support from a home care company, then you will have a care plan drawn up in conjunction with them, and at Helping Hands your local management team will assess you and devise your care plan with you. If you are planning to engage a carer privately then you may be planning to draw the document up yourself, but even then, you will no doubt talk to your wider community healthcare team about how best to do so.
