Autism Care Plans
What is an Autism Care Plan?
An autism care plan is designed so that someone living with autism can live as well as possible with all of the support they need in place. Being diagnosed with autism does not preclude someone from living a fulfilling and enjoyable life, but in some cases additional support is required.
Helping Hands can help our customers to live as independently as possible in the home they love. We’ll assess and manage your condition, as part of your wider healthcare team where appropriate, and make sure that your care plan contains all the information needed to help you live well.
Autism Care Plan Example
An autism care plan needs to contain all the information needed to help you live your best quality of life. This includes:
Assessment of needs
In as much detail as possible
Risk assessments
For managing behaviours effectively
Specific interventions
Such as occupational therapy
Why Should I Have a Care Plan?
There are many reasons why a care plan is important. These include:
1. Helping you to live well
The first thing that should happen after an autism diagnosis is that you receive a thorough assessment of your needs. This will ensure that you can have a care plan developed that contains all of the information to help you live well at home.
2. Respecting your choices
Your choices will always be respected and you'll be kept at the centre of your care. Effective care planning will ensure this happens and that your carers and other professionals know what you wish to happen on a daily basis.
What Does an Effective Care Plan for an Autistic Customer Include?
An effective autism care plan contains a lot of detail about your daily routine, including:
Personal Care
Such as when you like to get up, your showering or bathing preferences, how you like to wear your hair, whether you have favourite colour outfits, etc.
Meal preferences
If you like to cook for yourself and how much assistance you require, the types of food you enjoy and any allergies or intolerances.
Medication
If you take any medication to help manage your condition, the time of day, the route, and the quantity that needs to be administered.
Hobbies & interests
The type of activities you enjoy doing, how much support you need, whether you attend activities outside the house and when.
Community support
Whether you have other care professionals come to support you as part of a wider healthcare team, and when they come.
Personal budgets
How your care is paid for, whether you self-fund, get help from the local authority, and the type of help you get.
How Can I Create My Own Care Plan?
We keep you at the very centre of your care planning so we’ll always work with you – we won’t exclude you from your care planning decisions. Everyone involved in delivering your care will have an opinion on how best to proceed, whether that’s the local authority, care provider and other agencies, but it’s important that everyone works together to best design your care.
Helping Hands specialise in person-centred care; we have never considered a ‘one size fits all’ approach to be appropriate for anyone we support. We’ll get to know you and the way you like your days to proceed so that we can always make sure you’re living as well as possible in the home you love.
