Whether your regular carer is taking a break, you’re due to be discharged from hospital and would like some extra support in getting back on your feet, or you want to take a short break away but don’t feel confident going alone; our Southport team will ensure a seamless transition between your carers so that there’s no disruption to your usual routine.
